Medicaid Home and Community-Based Care is intended to provide services for those who cannot afford to pay for care with the goal of keeping the person out of a nursing home. Clients will first have to be assessed by State agency that administered the program and will approved the hours needed

PRIVATE INSURANCE-MCO-PHPs

We accept care Insurance policy, we can also help verify your benefit for home care services.

Personal Care Services provides services to assist the individual and/or family with essential home management and/or personal care tasks and/or family to remain, and function effectively, at home as long as possible

Community Alternatives Program for Disabled Adults

CAP/DA

This is a Medicaid waiver program for Home and Community based services that covers individuals with disabilities or seniors. CAP provides in home care, respite services and some personal assistance

Community Alternatives Program for Children

CAP/C

This is a Medicaid waiver program for Home and Community based services that covers children with complex medical issue between age 0 and 20. CAP provides in home care, respite services and personal assistance
